Insurance: Did you know that your health insurance may pay for you to see a nutritionist?

You may have 4 visits per year or unlimited visits up to your maximum medical nutrition therapy or prevention and wellness benefit limit. A referral from your doctor may be needed, however, most health insurance plans do not require a referral when you see a provider who is “In-Network.”

We are currently accepting the following health insurance plans (* indicates Preferred, In-Network provider with these insurance plans):

  • Aetna*
  • Blue Cross of Washington* and Blue Cross/Blue Shield
  • Cigna*
  • First Choice Health Network*
  • Please contact Group Health – Options, Alliant Plus, Options PPO, regarding your benefits, Options Select and Alliant Select require an authorization from your Group Health provider.
  • LifeWise*
  • Premera*
  • Regence*
  • Regence Boeing*
  • Tricare*
  • United Healthcare: UHC does not process nutritional counseling services. UHC contracts with Optum, a third party insurance plan. Optum offers a 20% discount for nutritional counseling, but they do not reimburse the nutritionist or patient. Passionate Nutrition honors this discount. Please complete the benefit advisor and contact your insurance directly with any questions.
  • Uniform Medical: (Uniform Medical will allow their members to see Regence providers for some conditions. Call Uniform Medical for more information.)

If you have medical insurance we are eager to help you receive your maximum allowable benefits.  In order to achieve our goals we need your assistance. Please fill out our Registration Form and Medical Insurance Benefits Request Form. Both of these forms are intended to help your billing process go as smoothly as possible. We appreciate your assistance and cooperation.

If your plan is not listed here please contact our insurance biller at 206.431.0138 to check on your nutrition health insurance benefits. You may be able to see us as an “Out-of-Network” provider and your insurance will pay a certain amount of the costs. We also offer discounted cash rates for those with Medicare, patients without insurance coverage or patients who prefer not to use their insurance for nutrition visits.

Medicare
We are not a Medicare provider. We are a “non-participating” provider with Medicare = Medicare will not accept claims for payment from non-participating providers. Medicare clients are offered a discounted cash rate and receive a detailed receipt created at the time of payment with each nutrition visit. The receipts can be submitted to Medicare and any secondary insurance providers for reimbursement. Reimbursement depends on the diagnoses being treated and are subject to approval by Medicare and secondary insurance companies.

Coverage for nutritionists varies between plans. Please check with your insurance carrier or our insurance biller for coverage details prior to making an appointment.

Rates are discounted and only applicable if paid on date of service. If using insurance, initial sessions are $225, and follow-up sessions are $144.
